Importantly, the analysis also provided reassuring evidence regarding a common patient concern: skin thinning with topical steroid use. The study found no increased risk of skin thinning when topical corticosteroids are used appropriately for short-term treatment (less than 16 weeks). Dr. Patterson emphasized that this evidence helps clinicians educate patients and dispel misconceptions about responsibly prescribed topical steroids.
The video also explores how treatment decisions are individualized. Dr. Patterson explains that factors such as the location of eczema, particularly in areas with thinner skin like the face or genital region, often guide clinicians toward non-steroidal options, such as tacrolimus. Severity and duration of symptoms, prior side effects (including burning or irritation with certain topical agents), and lifestyle considerations all play a role in determining the best therapy for each patient.
Dr. Patterson notes that not all PDE4 inhibitors perform the same in clinical practice. While some patients benefit from medications such as roflumilast – particularly in cases of seborrheic dermatitis – others may experience limited improvement or tolerability concerns with certain agents. These nuances underscore the importance of personalized dermatologic care.
Beyond medication selection, Dr. Patterson considers practical elements that affect adherence, including how often a medication must be applied, whether a patient is pregnant or lactating, and the formulation or “vehicle” of the product (cream, ointment, or lotion). These details can influence both effectiveness and patient comfort.
