Dr. Long Ly, a Board-Certified Dermatologist at our MetroDerm locations in Alpharetta, Atlanta-Sandy Springs, and Buckhead, was the subject of a recent Simply Buckhead article titled “Baby-Safe Beauty,” offering expert guidance on how expectant mothers can care for their skin safely and effectively during pregnancy.
Pregnancy brings dramatic hormonal shifts that often show up on the skin.
“The most frequent things I see with pregnant patients are hormonal acne, pigmentation from melasma, stretch marks, and dry, itchy skin,” says Dr. Ly.
While these concerns are common, treating them requires a thoughtful approach, as many traditional skincare ingredients and cosmetic treatments are not recommended during pregnancy.
- Acne
In the article, Dr. Ly emphasizes safer alternatives for managing breakouts, advising patients to avoid retinoids, high-dose glycolic or salicylic acids, and strong chemical peels. Instead, he recommends azelaic acid, available over the counter or by prescription, as a pregnancy-safe option to reduce inflammation, regulate oil production, prevent clogged pores, and improve post-acne pigmentation. Certain topical antibiotics, such as clindamycin or erythromycin, can also be safely used to reduce acne-causing bacteria.
- Pigmentation
Pigmentation, particularly melasma – often referred to as the “mask of pregnancy” – is another frequent concern. Triggered by hormones and sun exposure, melasma can be difficult to manage when bleaching agents such as hydroquinone are not recommended for use during pregnancy. Dr. Ly encourages diligent sun protection and the use of antioxidant vitamin C serums to support brightness and pigment control. “While it’s generally safe to use a chemical sunscreen, we don’t have the data about chemical sunscreens during pregnancy, which is why I recommend a mineral sunscreen,” Ly says, noting that zinc oxide or titanium dioxide formulas are preferred.
- Stretch Marks and Dry Skin
Dry, itchy skin is also common as hormone changes disrupt the skin barrier. “Hormonal changes can really dry the skin,” Dr. Ly explains. He advises choosing gentle, non-active cleansers and following up with thick, fragrance-free moisturizers after bathing. While stretch marks may not be preventable, consistent moisturizing can help keep skin supple, and post-pregnancy treatments such as laser or microneedling can improve texture and redness.
Dr. Ly also emphasizes the importance of maintaining routine skin cancer screenings during pregnancy.
“Hormones can make moles change, and while most are harmless, sometimes we do find skin cancer.”
he says, adding that biopsies and necessary surgical treatments are safe with local anesthesia and should not be delayed.
Finally, cosmetic injectables like BOTOX® Cosmetic and fillers should be paused until after pregnancy.
“Skip cosmetic treatments such as BOTOX Cosmetic and fillers until after pregnancy to avoid the risk for absorption of toxins and other substances into the bloodstream,”
Ly says, noting that it’s best to wait until the face has stabilized postpartum.
Above all, Dr. Ly reassures patients that skin changes during pregnancy usually resolve with time.
“Some women’s skin gets better, others worse,” he says. “These changes are temporary.”
